
Chronic and complex health conditions usually involve a condition that is present for 6 months or longer, and often come with multiple comorbidities. Research has shown that 1 in 5 Australians are affected by multiple chronic diseases. As these conditions don’t exist in isolation, optimal treatment for one condition may interfere with the optimal treatment for another and so will need to be managed by a team of health professionals. Such conditions include cancers, heart disease, stroke, obesity, type 2 diabetes, chronic kidney diease (CKD) and chronic respiratory diseases (such as COPD).
